QUALIFICATION PROCESS
If you or a family member is interested in services you can:

Fill out the form below, call a family support supervisor at 843-805-5800 or visit us at 1357 Remount Rd Unit #2

A family support supervisor will provide information about services and direct you to the appropriate screening provider. If your child is 0-36 months, call Babynet at 843-740-3193 to make a referral. For anyone over the age of three, call the SC Department of Disabilities and Special Needs (SCDDSN) at 1-800-289-7012 to make a referral.

After the initial screening, you will be given a choice of providers. We hope that you will select the Disabilities Board of Charleston County to assist in meeting your or your family member’s needs.
