A case manager will provide assistance with assessing needs, care planning, referral and linkage and monitoring/follow-up to identified needs. Case managers assist individuals in accessing services that are available through the SC Department of Disabilities and Special Needs and within the community.

Once eligibility through the SC Department of Disabilities and Special Needs is established, the case manager will complete an assessment and work with the individual and family to develop a support plan. The support plan is a working document that identifies the individual’s needs, strengths and goals.

Once goals and needs for support are identified, the case manager will help identify community and family resources to help meet those needs and goals, and will assist with referral to services, including activities to help link the individual with medical, social and educational services and programs.

Case Management is a program offered through the Family Support Services (FSS) department of DBCC. A case manager will provide assistance with assessing needs, care planning, referral and linkage and monitoring/follow-up to identified needs. Case managers assist individuals in accessing services that are available through the SC Department of Disabilities and Special Needs and within the community.

The case manager will monitor services to ensure that the support plan is effectively implemented, that it adequately addresses the needs of the eligible individual and monitor for progress and satisfaction.
